A copy of your birth certificate and player picture is required at registration.

You can upload that at the same time you register online.

This year we are accepting online payments by checks and credit cards/debit cards. If you

need to you can still mail your registration form and payment to 

SWV Fusion PO Box 747, Oak Hill, WV 25901.
